Why do I need to complete this form?

We ask for an Equality & Diversity Monitoring Form to be completed because Shape is committed to equality and diversity in employment and all aspects of its programme and operation. No person applying to work with Shape shall receive less favourable treatment on the grounds of gender, disability, HIV status, race, colour, nationality, ethnic or racial origin, religious belief, marital status, sexuality, age, responsibility for dependants or spent unrelated criminal convictions.

Who do I send it to?

Will my data be kept anonymous?

Yes. Please note it is important that you do not add your name to equality and diversity form to ensure our data collection is anonymous. If you wish to inform us about any access requirements you have for interview, this can be done separately. If you wish to identify as a disabled person as part of your application, for example, to be in the candidate pool for disabled people who meet the essential criteria, then please do this as part of your application or covering letter. We cannot use the equality & diverisry monitoring form to establish whether or not you identify as disabled. See our note on data protection below.

Because we recognise that people’s perspectives and experience of disability are very wide ranging, we have a broad and inclusive approach to what disability means. Some people will meet with access barriers on a daily basis; others may have a mental or physical impairment or health condition which is invisible to others or which they prefer to keep to themselves; some people may not even have considered themselves to be disabled until something has happened to make them reflect on the issue. If you consider yourself to be disabled under this broad definition, then please indicate so in the monitoring form and where applicable in your application.

Data Protection (GDPR)

Shape collects information for purposes related to your application and potential employment.  We may keep applicant details on file for a period of six months. We may check information provided by you, or information about you provided by a third party with other information held by us.  We will not disclose information about you to anyone outside Shape unless the law permits us to do so.
